Medical errors causing death, other problems rising in Maryland, report says

Medical mistakes that resulted in patient deaths or severe disabilities increased for the fourth consecutive year in Maryland hospitals, according to a report published Friday by the Maryland Department of Health.

In one case, a patient with chest pain sat in the emergency room for hours before being given medication a cardiologist had recommended to prevent worse heart problems. Because of the delay, the patient had a cardiac arrest and had to be transferred to another facility for higher-level treatment.
